I know this post was from a month ago, but I haven't been on here in awhile. My little girl was 6 lbs. 13 oz. at birth and always hovering around the 10% in weight. She had bad reflux, too, so we were seeing a GI doctor and he had us put oatmeal in her bottle as well as concentrate the formula to put on some weight. She really didn't start to gain well until after her surgery (at 7 months). At around 10 months she really took off and now she just turned a year and is over 20 lbs. She has become quite the butterball. She still doesn't want to eat baby food and we are struggling some with solids so we had an evaluation at the feeding clinic in our town. We are still waiting to go back for outpatient therapy but she will be seen by a OT/SLP, nutritionist, and behavioral psychologist. I did a similar thing for my other son who is also a poor eater, but had to stop because it was right around the time I delivered my little girl. Anyways, you should check into this type of program for your little one. Like someone else was saying, you can add high caloric things to food like butter and such, but I don't know about the lactose intolerance. My pediatrician also suggested adding some cereal/oatmeal to the baby food. They don't notice it too much. It just makes it a little thicker. You could probably add a little soy formula in there, too. I know even though my daughter is a year know and that she could start on whole milk soon, that the feeding clinic wants her to remain on formula a little longer for the calories/nutrients until she picks up her eating.
